Your first step
into coordinated care.
A 15-minute video assessment with a registered mental health nurse (not a form, not an algorithm) that becomes the foundation of your whole care plan.
A registered clinician,
right from the start
The intake call sets the direction for everything that follows. Your nurse listens to your story, assesses risk and goals, and builds a personalised plan, connecting you to the right combination of disciplines from day one rather than guessing.
- Wanting a considered start, not a quick booking form
- Unsure which clinician or service fits your situation
- Managing multiple needs: therapy, medication, practical support
- Starting care for the first time and wanting to get it right
What your intake covers
A structured conversation covering the territory that matters most.
Your story, your goals
What brings you to CareDirect, what you've tried before and what a good outcome looks like.
Risk and safety
A clinician-led safety check. Discreet, respectful and practical.
Medication & physical health
Current medications, conditions and what's relevant to your plan.
Social context
Family, work, finances and other practical factors that affect wellbeing.
Plan & matching
Your nurse recommends clinicians and services, and books the first appointments.
Clear next steps
A written plan, booking confirmations and your nurse's contact details.
How intake works.
Human-led, clinically structured, and consistent.
Step 1: Speak with a registered nurse
Your intake is with a registered clinician who can assess your needs from the first conversation.
Step 2: A private, unhurried conversation
Your session gives you space to explain what's going on. Longer sessions are available where needed.
Step 3: Leave with a care plan
You finish intake with a written care plan and a confirmed first appointment.
Step 4: Book without a referral
No referral is needed to get started. A GP Mental Health Treatment Plan is only required if you want Medicare rebates on future sessions.
Simple, transparent pricing for Intake & Triage
No hidden fees. Where you are eligible, we can help you claim any Medicare rebate that applies to your session.
Your intake is complimentary. There is no charge, and no referral is needed to book.
Questions about intake & triage
The practical things people ask most often before booking.
Yes. The nurse-led intake is complimentary. There is no charge for this session. For clinician sessions after intake, Medicare rebates may be available for eligible services where the referral, care plan and Medicare requirements are met.
That's exactly what the intake is for. Your nurse's job is to work out the right combination of clinicians, so you don't have to guess.
Nurse Triage operates Monday to Friday during business hours. If you submit a booking request outside these hours, your nurse will follow up with you on the next business day.
